Thanks again to Margaret, however the form would not send when the data
was typed in. I got a message "document contains no data" I tried
repeatedly and finally gave up (I emailed the update info to Thomas Hill
--which he may or may not welcome as the proper way to submit updates?).
Has anyone else had this problem?

There is an on-line entry/update facility for IDAL so may I suggest that
everyone look at it to ensure their entries are present and/or
up-to-date.
