If your are desperate our copy has been bound with the two images cut
off and stuck to the cover i.e. we could photocopy the two separately.
It has been covered in loose plastic to protect the image.  I cannot see
a signature.

By the way, we were not responsible for this desecration, we bought it
second hand!

It would need an A3 size copy i.e. snailmail not fax for full size.
