The PDF below from the School of the Art Institute of Chicago is a good consolidated guide.

Subject: [ARLIS-L] Faculty Artist Statements

All,

I have a Professor in the Visual Arts department who is wanting help rewriting her artist statement and bio.  I have located a couple of books, but am wondering if anyone has any valuable resources or examples of statements they have used with similar types of questions.

Thanks,
