In addition to the expense of indexing (not just the skilled labour, but extra paper) there's the time factor. Unlike a monograph that can go to press when it's finished, an exhibition catalogue needs to go to press in time to be delivered for the exhibition opening. Since there always seem to be last-minute changes (at our institution, anyway) there simply isn't time for indexing.
