Hello once again, please accept my apologies but I have just discovered
an error in the url I listed in my previous email. An errant period at
the end of the address makes it impossible to reach my survey. I also
neglected to mention that one needs to have cookies enabled in the
browser for the survey. Again my apologies! Please copy and paste the
correct url, which is as follows:
