I am in the process of trying to design a Docent Library Survey for a museum docent library. 

I am writing to inquire whether anyone on this list has experience designing such a survey and would be willing to share his/her survey instrument with me.  Of couse, I will give you and your instrument the appropriate credit in my bibliography for the survey.
